This morning, I hung up my shoes. I stepped down as a medical superintendent of the MGIMS hospital.
Twelve years ago, I became a reluctant MS of the hospital. For a physician-teacher who had not had a single day management experience, the initial days were unnerving, to say the least. Here I was—catapulted straight into test cricket. I was forced to make my debut at Sabina park, without having ever played first-class cricket.
The pitch was treacherous, the sky was overcast, some deliveries would suddenly bounce, and some deliveries would land wide of leg but spin so much that they knocked over the off-stump.
It took me some time to understand the game. Over the weeks, I realised where my off stump was. As months rolled by, I grew confident. I felt secure and seemingly at peace with myself. After playing in the “V”, I started playing my shots—hooking and pulling fearlessly.
I began to see a bigger picture and acted with the big picture in mind. Many problems begged for innovative solutions. Patients—today they have become increasingly empowered health care consumers—demanded more-convenient, more-effective, and less-expensive treatments. And health professionals demanded more staff, more budget, more equipment and more renovations in their infrastructures. They wanted—and rightly so—new drugs, diagnostic methods, innovative technologies, and medical devices that offered the hope of better treatment.
When the expectations are high, and the budgets are frugal, a MS has to walk a tightrope between the demands and the supplies. I was closely watched as I walked. I had my share of success. I had my share of failure. Not all my ideas panned out as I would have liked. And sometimes things turned out to be much better than I thought they would. In the process, I also got more bouquets—and brickbats—than I deserved.
As I look back, I realise that this responsibility helped me translate my several ideas into reality. I might be accused of blowing my trumpets loudly, but I do feel that at least partly, the hospital became more responsive to the needs of the community.
I might have been a bit petulant, sometimes taciturn, occasionally stubborn, unreasonable and demanding at times, but my colleagues kept on supporting me even on days when things seemed tough. I apologize for my shortcomings. I might not have scored enough runs when the pitch was friendly and the bowlers were bowling gentle full tosses. I thought they would be upset but they took my poor performance in their stride. But for their unconditional encouragement, I would not have lasted the tumultuous times.
Why did I resign? Many have this question in their mind but they are not letting the curiosity getting the better of them. Let me answer. I was getting stale. I wanted to leave the stage at the correct time. The time was ripe for me to hand over the reins to a zealous, brighter and blooming faculty in the institute—and, hopefully, a better MS. Time to pass on the baton to a younger pair of legs. I strongly felt that once I gave up the job, the change would surely boost the bottom line of MGIMS—in more ways than one.
These changes, I felt, were not simply nice to have, they were crucial for the future success of MGIMS. As Alfred Tennyson once famously wrote, “The older order must change, and yield place to new.”
Wasn’t it Vijay Merchant, the Indian Cricket Captain, who had put it succinctly in 1948, “Retire when people ask why and not when.” Realisation dawned on me that what holds true in cricket also holds true in healthcare.
My heart is full of gratitude and respect for the institute I worked for. For decades, my colleagues and my staff believed in me, gave a short shrift to my weaknesses and seldom breathed over my shoulders. They struck together through thick and thin. They give me free reins to run the hospital.
How do I sum up my MS days? I can look no further than Charles Dickens whose 1859 novel, A Tale of Two Cities describes the scene in London and Paris:
“It was the best of times, it was the worst of times, it was the age of wisdom, it was the age of foolishness, it was the epoch of belief, it was the epoch of incredulity, it was the season of light, it was the season of darkness, it was the spring of hope, it was the winter of despair.”

This morning, when they assembled to bid a teary farewell to me—telling me what I meant to them—their kindness left me moved and grateful. I couldn’t find enough words to thank them for their unstinted trust.
What next? Many lips had this question this morning. Now that the rigmarole of administration is behind me, I will focus on teaching and caring for patients—focusing on those who need palliative care. I’ll pick up the research thread. And I plan to spend more time on reading and writing—my lifelong hobbies. More time for my family, particularly for Diti and Nivi, and Krit, my grandkids.
I hope that this change will help me find a clear sense of purpose. Make my mind calm and tranquil. And give me space to rediscover myself.
Among the several changes, two area, the HIS and Pharmacy, were the significant which lead to smooth functioning of hospital. And your simple but elegant designing ideas are visible in all new buildings. Congratulations for a marvellous tenure as MS and best wishes for future.

I am writing this email to express my heartfelt thoughts.
My sincere appreciation and wishes for a masterclass of an innings.
One of the things that I learnt from one of my Mentors was that assessment should be based on – what was the condition of the Team/Dept/Organization when you joined and what is condition when you part from it. Between these two reference points, you have left an indelible mark and legacy through your work. Be it low cost drug initiative, the hospital information system, facilitating learning at the point of care through use of technology, advocacy for evidence based care, establishing state of the art departments of Medicine and Palliative care…… to name a few. All this while delivering compassionate care and mentoring hundreds of students.
The role modelling and the leadership that you demonstrated during the Covid pandemic is simply unparalleled. The humanistic and bold decision of allowing the relatives to meet the covid patients (I am not sure how many hospitals did that), leading the district task force, constant advocacy for rationale use of Hydroxychloroquine, Remdesivir, Favipiravir, Tocilizumab, Methylprednisolone, N95 masks for that matter, including confronting our own staff on many occasions. The way you balanced the hard and soft nature of being an administrator is worth learning.
Also I admire you for your decision to call it a day when people still wonder ‘why’ and not ‘why not’. It’s not easy to walk the talk and many succumb to the temptation of lingering on.
I sincerely wish that you pen down the wisdom from all these years as these will truly be life lessons in itself.

We humans fear the unknown and are comfortable with continuity /certainty. This makes it very difficult to take such decisions, especially when in control of things. And only a person with lot of wisdom can take such decisions even when it is not forced. Hence your decision does not come as a surprise as you are one of the most wise people that I have come across. This decision further enforces that belief.
I know it must not have been an easy decision. Having spent all the prime years of life in this role, it must have been quite emotional too. But I am sure you are more than capable of handling this transition. That clarity reflects in your retirement post too.

Because going through those lines I really felt it was a very personal letter you addressed to one and all in MGIMS. Your letter shows how transparent, open, free, humble, dedicated and other oriented you are.
First of all let me acknowledge you very frankly that I admire the way you do things, the way you manage every situation with calm and serenity. You have a solution for all the problems. The sudden change has really come as a surprise. The administrator has to walk a tight rope most often. ‘you cannot satisfy everyone, is the dictum. There will always be some people who try to see ‘something fishy’ in everything, you have very aptly dealt with the administrative responsibilities, focusing on the ideals of the institution and needs of the society.
There are always limitations and restraints because of many factors related to the finances, personnel and some knows/unknown strings and I am happy that you have been able to do the justice despite all of these.
You are welcome back to your original house—the department. Teaching, research and hobbies –more time will be available to fulfil the desired objectives.

And very aptly, you have invoked the analogy of cricket, a group sport, something that you love very much (I guessed this from your tweets :). Managerial jobs are group games, something we physicians are not very well trained in. We are at most doubles badminton players.
You made the letter look very light but it is deeply philosophical. It reflects upon the complexity of the real world and stresses the need to be humble and accept shortcomings.
In these rapidly moving times where the rate of change of change is very high, one wonders what is the right time to hang boots and whether it is going to shift to earlier ages.
You end on the note- what next ? Such a profound question ! For someone who is always immersed in work and always seeking purpose, this could be an unnerving question. But I guess one has to rediscover oneself every second of our life and keep figuring things out.

Sir, this delayed response is because I have been trying to collect my thoughts ever since I heard the news of your resignation.
MGIMS, and Sewagram, is where my roots are. The place is defined by its people, and you, in many ways, have been the face of the institution for more than a decade. For those of us who have been your students, that was a reassuring thought. More so for me. Though this must have been a considered decision, I had mixed feelings. The thought of MGIMS without you as Medical Superintendent is, in all honesty, not a palatable one. MGIMS has grown in many ways during your tenure, and we owe a debt of gratitude to you for that.
However, as you very succinctly put it, I suppose it was time to reclaim your life. I do empathize with that. It is the start of a new innings for you. I’m working as Assistant Professor of NEIGRIHMS , Shillong. We are having MBBS course for 50 students and two postgraduate students in each batch . I’m looking after the 15 bedded medicine icu and quite busy with administrative work apart from clinical and teaching curriculum . But with the teaching and experience I received from MGIMS and CMC ,Vellore , I always feel that nothing is difficult .

Regards. It has been a long and memorable inning with you as the MS (that started roughly around my internship). It was one of the many hats that you donned over the years while inspiring us to take up challenges and take roads less travelled.
The Tweedledum Tweedledee episode that rocked the Orthopedics Department was one of the first instances where I realized how an administrator, physician, and professor deals with errors. Also, the successful NAAC accreditation and your simile of the entire endeavour to be “like a daughter’s wedding” is memorable. Your help with the point-of-care device trial that I worked on, was also during this golden age of 12 years. In these 12 years, Sewagram changed, the world changed. The buildings, the students, the internet, the satellite news trucks, the doctors, the students, the patients, every thing changed. And yet like a seasoned batsman, you guided Kasturba Hospital to become a better place while standing up for what you believed in. As you raise your bat towards the crowd and walk towards the pavilion, with a standing ovation I salute an inning well played that will inspire future generations.
Now coming to a lighter note. While daydreaming a couple of days back, I was wondering what will a slide deck of my life story look like. I came to an instant conclusion that the introductory slide will be a quote saying “It’s not the destination, it’s the journey” with the name Ralph Waldo Emerson scratched out and Prof. SP Kalantri written on it. That is how my career in research started and I may be one of many who have similar stories to tell. That is the moment when I started worshipping the art and the artist as you sculpted your David and painted the Sistine Chapel.
Coming back to the reality that stood in front of me, I was composing an email (a ‘stinker’ as they call it in the quagmire of corporate politics). While I tried to bring in Longfellow, the footprints, the hurt, and the unbecoming that needed to be put into the email, the end result was laughable when compared to the elegance of this pdf simply named “SP_Resignation”. It is a lesson on how graceful can a wordsmith be, no matter what the circumstances or the backdrop.
As a final thought, being from Nashik, we are used to seeing the spring of hope (and development and celebrations) every 12 years and hence we love that number. This is no different. There are better things that are coming and like an ardent follower, I am waiting in the stands- for the second inning, for the player of the match, for the celebrations, and for yet another wave towards the stand.
One who didn’t work with you at times of pandemic will never know how one can guide and inspire ethical conduct the way you did.
Sir, many congratulations for a long and successful inning.
This has come as most unexpected and surprise for me. You have carved your own space on this prestigious institute with your hard work , zeal and commitment. You have raised the benchmark of academic excellence and patient care. Hundreds of Doctors and other medical fraternity persons are walking on the illuminated path of yours. As usual, a highly enjoyable essay about your retirement from management (can’t imagine you will ever retire from looking after patients and teaching/research!). I particularly enjoyed the analogy with cricket since in the last few years, mainly to play with my son, I have taken it up again after a gap of at least 40 years.
I had seen the news of Poonam taking up the role of MS, in the 1982 batch WhatsApp group. But did not know that she was replacing you! I can imagine guiding the vast ship of MGIMS over the last few years during Covid must have also taken its toll. Your shoes will be difficult for Poonam to fill but at least she has the advantage of being able to depend on you for your sagacious advice when needed.

No words will be sufficient to “ repay” what, I believe, the collective owes you- students, staff, alumni, management. For as long as I can remember my visits to my alma mater, over the past 15 years, you have been an enthusiastic, driven force enabling the beautiful unfolding of the transformation of MGIMS. I remember how excited you were is sharing the plans for so many of these monumental changes- the old library conversion, the old gynae/ peads conversion, going paperless when no other major med school was thinking of one, so on……. But yes, your decision is another exciting phase of your life. Looking forward to meeting an even more “chilled out” Dr SP on my next visit💐💐💐💐It is great that you will remain an active clinician and teacher💞💞

The news is really indigestible that you are leaving, not just because you are elder than me and hold a special place in my heart but also because you are the true motivation for me and even my other MGIMS colleagues. I feel sad to hear that you are leaving because whenever I reached you with my sad face you made me smile by helping me out every single time and supporting me in all my tough times.The positive part of this news is, though you are leaving your post but you will be there with me in this department. At last I would just like to add one thing that though you are leaving your post, you will never leave our hearts. Hats off to your work sir you have another level of potential in you.

Although we were not there for a major part of your tenure but we were always proud to have you at the helm of affairs.
