But he walked it anyway — and then the one to Medshi, and the one to Harisal in the Dharni forests of Amravati district, and several others after that, across four districts, over more than three decades in government service. When he finally retired in 2012, he had seen more of rural Maharashtra from the inside — its PHCs, district hospitals, sub-district wards, and leprosy centers — than most doctors in the state.

He had also, somewhere along the way, obtained a diploma in ENT.

From Balkhed to Akola

Ganeshgir was born in Balkhed, a village in Washim district, to a father who was simultaneously a tailor and a farmer — a combination common enough in rural Maharashtra, where one income source rarely sufficed. He walked five kilometres to school each day, crossing a river on one side and open sewers on the other. He remembers the route with clarity. What he perhaps did not anticipate, crossing those fields as a boy, was that he would spend his professional life in circumstances not entirely different: moving through poorly connected towns, serving populations that depended on whoever the government sent.

He attended Tilak Rashtriya Saraswati Mandir school in Umari Akola through to his eleventh standard, then enrolled at Radhakisan Laxminarayan Toshniwal College of Science, Akola for his pre-medical education. In 1973, he joined GMC Nagpur.

His early years in Nagpur were peripatetic. He stayed first with a batchmate at Hanuman Nagar, then at his uncle’s home in Raghuji Nagar, and finally found his footing in the GMC hostels. The college, with its overlapping cultures of discipline and chaos, its hostel kitchens and late-night study sessions, shaped him in the ways it shaped everyone who passed through it.

Government Service: The Long Road

After MBBS and internship at the primary health center in Rohana and the district hospital in Akola, Ganeshgir joined the ESIS Hospital in Akola for two years — a decision shaped partly by the fact that government medicine was what was available and partly by a disposition toward public service that his upbringing had instilled. He then moved through a series of primary health centers in Akola and Amravati districts: Jamthi near Murtijapur between 1983 and 1985, Medshi between 1985 and 1986, and Harisal in the tribal belt of Dharni, Amravati district, between 1988 and 1990.

Harisal is in the Melghat region — one of the most nutritionally vulnerable areas in India, a forested belt where child mortality remained stubbornly high for decades despite government intervention. To practice medicine in Harisal in the late 1980s was to work with almost nothing: limited drugs, unreliable supply chains, patients who walked for hours, and a health infrastructure that was designed on paper to serve them but often failed in practice. Ganeshgir served there.

In 1990, he was transferred to the district hospital in Akola, and thereafter kept moving: district hospital Akola between 1991 and 1995, Shegaon between 1997 and 1999, Amravati between 1999 and 2000, Akola again between 2000 and 2006, Chandrapur between 2007 and 2011, and finally as Additional Director of Health Services at the Akola leprosy center between 2011 and 2012. He retired on 30 April 2012.

In between these postings, and as an in-service candidate, Ganeshgir obtained a Diploma in Otorhinolaryngology (DLO) from Indira Gandhi Government Medical College, Nagpur, between 1995 and 1997. The decision to acquire a specialist qualification mid-career, while continuing to serve in public health postings, reflects the practical intelligence that characterises the best of the GMC 1973 generation: they took what the system offered, augmented it where they could, and kept working.

The Texture of a Public Health Career

It is worth pausing on what a career like Ganeshgir’s actually involves. The public health physician in Maharashtra, posted to a series of district and sub-district facilities, rarely appears in the medical press. He does not publish. He does not present at conferences. He does not accumulate citations. What he does is show up, day after day, in facilities that are understaffed and undersupplied, and provide whatever care he can to whoever arrives.

The patients who came to Ganeshgir across his career came from cotton-farming communities, from forest villages, from industrial townships. They came with tuberculosis and malaria, with complicated deliveries and snake bites, with the full range of disorders that Indian public medicine is asked to address. He saw them, treated them, referred what he could not manage, and kept the facility running as best he could.

When the government wanted someone at the Akola leprosy center in his last posting, Ganeshgir went. Leprosy work — diagnosis, treatment, surveillance, the slow reduction of stigma — has never been glamorous. It has been, in the hands of steady public health physicians, quietly effective.

The Anandibai Joshi Award, given by the Maharashtra government for outstanding work in primary health care, was not designed for doctors who seek attention. It was designed for doctors like Ganeshgir.

After Retirement

Following his retirement, Ganeshgir spent six months as a volunteer at the Regional Police Training Center in Akola — a small, unremarked act of continued service — before beginning a private practice that draws on both his general medicine background and his ENT diploma. He sees patients with ear, nose, and throat complaints alongside the broader spectrum of general medicine: hypertension, diabetes, respiratory infections, the chronic conditions that form the bulk of outpatient practice in any Indian town.

His daughter Sonali followed him into medicine, obtaining an MBBS from GMC Nagpur and a DGO from GS Medical College, Mumbai. She married Dr. Sanjay Bharti, an MD in Chest Diseases, who runs an ICU at ICON Hospital in Akola. His other daughter, Dipali, pursued a master’s in physics; his son Avinash chose electrical engineering and now teaches mathematics at an entrance coaching institute in Akola.

The family is rooted in Akola — the same city Ganeshgir walked to school in, the same district where he served his first government posting, the same place he returned to when his government career ended. There is a completeness to this geography.

The road to the primary health center in Jamthi, near Murtijapur, was not the road Ganeshgir Gosavi had imagined for himself when he entered Government Medical College, Nagpur in 1973.
